FinalForms Registration

FinalForms Registration

We are very excited to announce that the Hartland Consolidated Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.  You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

 

Use the Parent Playbook to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support”.  Once registered you must submit a completed Physical Evaluation Form and complete all other online forms.

  • In order to tryout, you must have a current physical on file in your Arbiter Athlete account.
  • Your current physical must be dated after April 15, 2023.
  • All information must be filled out on the form including multiple signatures from parents/guardian and player.
  • Submit this completed form to your FinalForms account before tryouts.

Pay-to-Participate (P2P) Fee

Pay-to-Participate (P2P) Fee

  • You will receive a bill from HHS after Spring Break.  Please plan to pay promptly!

 

  • The current Pay-to-Participate fee is $175 per sport.
  • To pay online, use your already created Parent Portal account.
  • If you need financial assistance please fill out the P2P Scholarship Request Form for either a full or partial scholarship or to request a payment plan and turn it in to the Athletic Office.

  • The MHSAA has adopted a concussion protocol that requires an M.D., a D.O., a Physicians Assistant, or a Nurse Practitioner to release to play any athlete who has suffered a concussion.  The form below should be used to provide that clearance.  It should be turned in to the Athletic Trainer.

Transportation Waiver

Transportation Waiver

  • A Transportation Waiver is required when you wish to be released to a parent other than your own or when you will not be using the bus for transportation.  This form needs to be turned into the Athletic Office as it requires the signatures of both the AD and your Coach.
